A sustained relationship with regular check-ins, consistent oversight, and proactive coordination. Best for complex or evolving situations requiring continued attention.

Consultative Engagement

Focused support for a defined period or specific challenge: a care transition, a crisis, a second opinion on a care plan, or a family navigating a new diagnosis.

Immediate response when situations escalate. We step in, assess, and coordinate the response across medical, family, and facility systems until stability is restored.

Active management of hospital-to-home or hospital-to-facility transitions. We coordinate with discharge teams, verify follow-up care is in place, and monitor the transition through completion.

Evaluate home care needs, assist families in selecting appropriate services, and serve as a care monitor to ensure quality and continuity. When home care is the right bridge to recovery or long-term quality of life, Adriel helps make it work.

Help families evaluate and select appropriate levels of care and residential options, including assisted living, memory care, skilled nursing, and alternatives to institutional care.

Coordinate essential services delivered directly in the home — including phlebotomy, podiatry, dental, x-rays, and other clinical services. For clients who face challenges with transport or prefer the convenience and dignity of home-based care, Adriel identifies and arranges the right providers.

Identify the right therapeutic interventions — cognitive therapy, physical therapy, speech therapy, occupational therapy, and nursing — and connect clients with the appropriate providers. For those who prefer concierge-level or in-home delivery, Adriel coordinates the sourcing, vetting, and ongoing oversight of those services.

Comprehensive evaluation of medical, functional, cognitive, and psychosocial needs. We develop a coordinated care plan and update it as circumstances evolve.

Attend appointments, facilitate communication between physicians and specialists, monitor adherence to medical orders, and ensure nothing is lost between providers.

Ensure medications are being taken correctly, on time, and in the right dosages. Monitor for interactions, reconcile medications after hospitalizations, and coordinate with prescribing providers when adjustments are needed.

Manage the logistics of a complex care calendar — scheduling across multiple providers, reducing duplication, ensuring timely follow-up, and making sure nothing gets lost between appointments.

Specialized navigation for individuals with Parkinson’s disease, stroke, traumatic brain injury, multiple sclerosis, dementia, and other complex neurological conditions.

Coordination and navigation for individuals with serious mental illness, psychiatric crisis, or co-occurring medical and behavioral health needs.

Keep all relevant family members and professionals informed. Facilitate family meetings, clarify medical information, and provide guidance on how to support a loved one through complex situations.

Facilitate access to meaningful activities — recreational, cultural, and social — that support cognitive health, emotional wellbeing, and overall quality of life. Particularly important for individuals with neurological conditions where engagement and stimulation play a direct role in outcomes.

Assess the home environment for safety risks, recommend appropriate technologies and modifications, and monitor for signs of exploitation, abuse, or neglect. Adriel serves as an informed, watchful presence that keeps vulnerable individuals protected.

Compassionate guidance during the final stage of life, including hospice coordination, family support, and ensuring the individual’s wishes are honored.

Navigation and advocacy when legal and medical systems intersect. Adriel works alongside attorneys, courts, and families in guardianship and protective proceedings.

Connect families with elder law attorneys, assist with navigation of federal and state entitlement programs, and provide informed guidance when legal and medical systems intersect. Adriel draws on legal training to help families understand their options and access the support they are entitled to.

Help families navigate the complexities of insurance coverage, prior authorizations, and medical billing. Adriel works to ensure clients are not left managing these systems alone at the moments when they are least equipped to do so.

For established clients in active care management engagements, Adriel is available around the clock. Complex situations do not follow business hours, and neither do we.
